Whether you're requesting a campus extension or cell phone
Name of individual receiving the new line
Delivery location
If campus extension:
- Extension they will be using (if known)
- Preferred headset style (see ITS Recommended Phone Headsets (FAQ) for choices)
If cell phone:
- Type of device
- FOAPAL
