What to Expect
Human Resources will review the submitted questionnaire and may contact your department for additional information or clarification before making a classification determination. You'll be notified if any additional documentation or next steps are required.
Requirements
In order to request help, you'll need to provide the following information:
- The name of the individual or business entity providing services
- A description of the services to be performed
- Proposed engagement dates
- Information about supervision, work expectations, and payment arrangements
- Details about the individual's relationship with the University, including any prior employment history
- Approval information from the department authorizing the engagement
