Telephone Request

Request a new telephone for a new or current employee.

What to Expect

An ITS staff member will reach out to schedule a time to install and activate the new phone.

Requirements

In order to submit a service request a new phone,  you will need to provide the following information:

  • Name of individual receiving the phone
  • Extension for the person receiving a new phone (if known)
  • Location for the new phone, including building and room.
  • Phone options needed such as: caller ID, speakerphone, or multiple lines
Additional Information

The ITS staff member handling the request may not have a key to all office locations, so please make sure someone is available to let the technician into the location where the new phone will be installed.
